Cardiology Patient Navigator

About the role

The role centers on patient communication, scheduling, care coordination, and clinical documentation. The ideal candidate brings a clinical background, experience in telephonic or remote care settings, and comfort managing a high-volume caseload of 150+ patients across multiple priorities. Cardiology-specific experience is preferred but not required.

What you'll do

• Patient Communication & Scheduling
• Manage patient scheduling, reminders, and follow-up workflows in EMR/EHR systems
• Conduct telephonic or remote patient outreach to reinforce care instructions and coordinate next steps
• Support patients navigating cardiology care pathways, including CHF management protocols
• Track testing schedules, follow-up appointments, and remote patient monitoring workflows
