About the role

The role centers on patient communication, scheduling, care coordination, remote monitoring alert triage, and clinical documentation. The ideal candidate brings a clinical background, an active nursing license (RN or equivalent), experience in telephonic or remote care settings, and comfort managing a high-volume patient caseload across multiple priorities.

What you'll do

• Patient Communication & Scheduling
• Manage patient scheduling, reminders, and follow-up workflows in EMR/EHR systems
• Conduct telephonic or remote patient outreach to reinforce care instructions and coordinate next steps
• Support patients navigating cardiology care pathways, including CHF management protocols
• Remote Monitoring & Care Coordination

What you need

• 3–6 yrs of experience
• Coordination
• Documentation
• ICU care
• Vital signs monitoring
• Any graduate
